 Advance practice nurses (APN) face varied nursing challenges daily. In most instances, ethical principles come to play, leading to ethical dilemmas. In this scenario, for example, teenagers and a parent are involved, and it is crucial as an APN to appreciate their developmental characteristics and the present context. The necessary information necessary for health assessments and diagnostics tests is discussed. Besides, consideration is given to appropriate responses to this clinical context as an APN.

The first clinical step would be to obtain subjective information from the parent and the patients. I would ask the parent why she thought the pelvic exam and birth control is necessary for her children since she is ethically obliged to decide on behalf of her children (Coughlin, 2018). This would help me acquire the teenagers’ medical history regarding health issues related to pelvic exams such as vaginal cysts, heavy and painful bleeding, endometriosis, or unhealthy sexual behavior. This would be crucial in identifying whether the parent and the children are knowledgeable about contraception to make appropriate, safe, and healthy decisions.  Besides, I would be able to identify the appropriate and targeted diagnostics tests for the pelvic exam.

Consequently, I would have the parent leave the examination room. After which, I will interview the patient individually since they are teenagers and can answer basic clinical questions with assistance. It is crucial to review the risks and benefits before performing the pelvic examination behind a closed door or curtained room to enhance privacy (Pozgar, 2019).

From the interview, I would ask questions related to their sexuality, including their knowledge of safe sex practices and contraception, whether they have had sex, menstrual flow, or problem with their menstrual cycle, if any.  As an ANP, I would handle each teenager independently and with dignity, constantly asking for their consent for each exam carried out. In this sense, I would be observing their rights and the principle of informed consent.
