Abstract
The provision of timely, empathetic, and appropriate care is critical in emergent care. Patients who leave without being seen (LWBS) in ED are at risk for worsening morbidity due to delay in diagnosis and treatment. In particular, high LWBS negatively impacts several domains of quality, such as timeliness, safety, and patient-centeredness. The purpose of this quantitative quasi-experimental DPI project is to establish to what degree the implementation of clinical inter-professional team-based in Triage reduces emergency department LWBS rates when compared to RN only triage within a 4 to 6 week time period.
The study utilized the Health Belief Model and Lewin’s change theory to examine the problem and verse the implementation of the proposed change. A quantitative retrospective design was to collect data and compare the benchmark times before the CITiT program during times of high patient census during the weekdays. The sample of the population was of RNs (n=16), at a 247-beds Broward County Hospital facility. Patient Perceptions of Quality (PPQ) instrument was administered to a sample of 60 patients identified and recruited after receiving care at the facility.
The statistical significance level at a two-tailed p-value of 0.05 was set for all outcomes. A t-test showed a significant decrease in the LWBS rate (M= -1.86; SD=1.41; p=0.001), decrease in PWT (M=-2.86; SD=1.29; p=0.003), and an increase in patient satisfaction rates (n=54) (M=-2.06; SD=1.29; p=0.003). A clinical interprofessional team in triage decreases the door to physician-patient wait time, thus accelerating the delivery of care during times that experience heavy patient volume. It is imperative to examine the impact of CITiT in a larger ED setting over a longer period of time.
